These forms will guide you through the basic introductions of client intake and consent for counseling treatment in healing and growth. Please answer all questions and sign all applications, including the HIPAA privacy notice, making sure that your contact information is correct. Once received, I will review the completed applications and contact you. I have a deep belief in working with the entire person and respect my client’s rights to privacy.
